To be clear, this is a hypothetical trade. I have no reason to be believe this will actually happen...although I'd like it if it did.

So the basic trade is Nash+Gortat for Horford+Joe Johnson
Obviously salaries need to be matched so the suns throw in any 2 out of Grant Hill/Josh Childress/Channing Frye/Hakeem Warrick

For example:

You could also throw in Teague or ATL's 1st if you wanted to sweeten the pot for Phoenix. Or replace Horford with Smoove but I'd rather have Horford if I was the Suns; especially since it would give me a better pick in the draft.
